f) To request that the actions taken pursuant to subsections (d) and (e) be notified to third parties to whom your personal data has been transferred,
g) Object to a result arising to your detriment due to the analysis of your processed data exclusively through automated systems,
h) If you suffer damages due to the unlawful processing of personal data, request compensation for such damages.

Your first and last name and, if the application is in writing, your signature,
For citizens of the Republic of Turkey, your T.C. identification number; if you are a foreigner, your nationality, passport number, or, if available, your identification number,
Your residential address or workplace address for service of notice,
If any, your email address to be used for notifications, telephone and fax numbers,
The subject of your request,

must be included, and if available, the information and documents relating to the subject must also be attached to the application.
